I am just fully recovered from a bilateral mastectomy I have lost so much ability in my upper body what can I do to get the strength back with out feeling like my scars are going to rip open. I am having a really hard time with push ups planks and things of that nature

Maybe just give it some more time?

Otherwise, you can regress push-ups quite a bit, planks similarly so, by raising the surface your hands rest on they get easier, so a bench, a bed, a counter, a wall, etc... or using a light resistance band, wrapped around an anchor point or even just behind your own back under your arms, you can grab the ends and do chest presses with it. Bands are extremely versatile and very popular for rehab work.

I would recommend asking your doctor to recommend some exercises and perhaps a general approach as far as time and progression/recovery expectations.
