LF Program Advice for easing back in post-kids, various injuries

Need some help here. Here's my current state, 5'8, 35-40 lbs overweight, herniated discs l5,l4, s1, intermittent right shoulder bursitis, inflamed right Achilles. Weight lifter in the past, yo yo dieter. Other than walking/light hiking, I'm not sure how else to ease into getting back in shape. Diet and core strength probably the best place to start. Gym membership currently sitting unused. Have the motivation but lack the energy what with 2 little ones running around. Appreciate at tips. Cheers and thanks!
