You got this. Just make a plan and stick too it. I know it is easier said than done but if you can put up what exercise you do, I will put in the effort to match it and then we can help each other stay on it!

Why do you want to be fit and healthy? 

Live longer?
Get a girl?
Protect you children?
Protect youself?

Whatever it is amplifie that in your mind. ..think possible senariio and start now or never will..  good luck

watch a kung fu movie. some of my favorites are Iron Monkey, Hero, Ip Man, and Fearless. Find some energizing music too and make sure you eat healthy: lots of fresh raw organic fruits and vegetables!! that will help to give you the energy you need.
